Welcome to the Tuscarawas County Health Department Birth and Death Certificate online ordering system.  Our office hours are from 8:00 am to 4:00 pm Monday through Friday. 

GETTING A BIRTH OR DEATH CERTIFICATE

 

Birth Certificates 

  • This office only issues certificates for births that occurred in Ohio from December 20th, 1908 – present. 
  • For births outside the State of Ohio, please contact the state in which the event occurred.
  • Requests for more than one individual must be submitted separately. 

 

New Birth Certificates 

  • Certificates for new births typically take up to 2 weeks to become available. While Tuscarawas County Health Department strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into the State of Ohio records system.
  • The Office of Vital Statistics processes orders within 2 business days once the information is available.

Death Certificates 

  • Death records for a person who passed away in Ohio between 1908 and 1970, you will need to contact the county where the death occurred to get a copy of the death record. For deaths that occurred anywhere in Ohio from 1971 to the present, you can obtain a certified copy of their death record from us, or from any local health department in the State of Ohio. Feel free to reach out to us or your local health department for further details.
  • For Death records in Tuscarawas County prior to December 1908, contact the Tuscarawas County Probate Court at 330-364-8811 ext. 3266.
  • For Death records in any other county prior to 1908, contact the probate court of that county. 
  • For deaths outside the State of Ohio, please contact the state in which the event occurred.  
  • The year of death is mandatory to obtain a Death Certificate. If you do not know the exact date, please enter (99/99/2000).

 

Attention Applicants 

  • If you are applying for your own Birth Certificate and you have changed your name since birth, (Marriage, Legal Name Change, Adoption) please enter the reason for your name change in the 'Special Instructions' field on Step 4 of the application.

 

Social Security Numbers on Death Certificates
Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy. Relationship restrictions apply and acceptable proof is required; for details, view the Social Security Number Guidelines and Addendum Form (PDF), effective October 15, 2015.
